Where Do I Start?

If you are thinking about exploring Long-Term Care options and the care you will need in the years ahead, Ontario Health at Home can let you know what your options are. You can apply for Long-Term Care by contacting Ontario Health at Home and a Care Coordinator will arrange to meet you to complete all of the required assessments.

Service Description:  Health care services offered at home, at school, and in the community, as well as access to supported living programs and long-term care. Care Coordinators work with people in the community or leaving the hospital to determine what help is needed and arrange for care.

Services may include:

  • Assessments for Long-Term Care Homes
  • Access to supported living (adult day programs, supportive housing, retirement homes and assisted living)
  • In-home support services (Nursing, PSW, Physiotherapy, etc.)
